The Board congratulated the Queen Elizabeth Hospital for recent achievements in the press, including the Maternity Service’s ranking as the highest quality in England and winning an award for work done on the prevention of diabetes.

It was also noted that OFSTED are currently inspecting the Council’s Children’s Services.

Minutes:

The Board received a presentation updating on the progress so far of the 10-year National Women’s Health Strategy, published in 2022.

The presentation recapped the national priorities for women’s health and noted that a top priority for women in Gateshead has been the continued work on Women’s Hubs.

A needs assessment, led by North East Office for Health Improvement and Disparities, has been developed using existing women's health data profiles, women's voice intelligence taken from a variety of local sources from across our health & social care and VCSE partners and from mapping of current commissioned women's health services to identify gaps with existing provision.

The Women's Health Programme has been developed to deliver the priorities for Years 1 and 2 through the development of an implementation plan, which will contribute to the longer-term outcomes contained in the national long-term strategy.

The progress on this is as follows:

Year 1 (2024/25):

  • Women's Health Hubs [which will include a comprehensive evaluation].
  • Contraception including LARC [long-acting reversible contraception] and postnatal services.
  • Menopause.
  • Cancer.

Year 2 (2025/26):

  • Ageing Well and Long-Term Health Conditions.
  • Violence Against Women & Girls [VAWG]
  • Fertility.
  • Mental Health.

The Board was given an in-depth update on the progress of Women’s Health Hubs.

This covered the core services, a model of care, and the variety of hub models that have been considered. It was noted that Women’s Health Hubs will also be linking up with Family Hubs to support coordinated service delivery. The model of care was outlined as follows:

  • Gateshead Place
  • Improving access and outcomes for women and girls
  • Focus on areas of deprivation
  • Multi-agency steering group overseeing the work
  • A shared and collective vision
  • Non recurrent investment secured to end March ‘25
  • A sustainable model

A survey was conducted, to inform the scope of the work and particularly to address whether the three current priorities were still in line with public opinion and what people’s experiences of NHS support are. The survey had 1001 responses, which had consistent demographic profiles with the 2021 census data for Gateshead, with a note that younger women were slightly underrepresented.

The headlines from the survey were as follows:

Menopause                             Almost half of respondents (48.7%) did not feel informed about menopause before they started experiencing symptoms.

For those who said they had experienced difficulties with support or treatment around menopause, the top answer was “lack of specialist knowledge within GP practice” (42.6%). 30.9% said “dismissal of symptoms”

Sexual health Services           For the 23.0% who found it difficult/very difficult to access, reasons include “length of wait/other access issues” (60.7%), “service not available at GP practice” (32.1%), “limited appointments” (28.6%), “issues with telephone only appointment system” (21.4%).

                                                The majority of respondents (59.7%) had never used a Long Acting Reversible Contraception (LARC)

Gynaecology Services            94.7% of respondents highlighted gynaecology as being very important or important

Long waiting times (30.1%) and being discharged without a resolution or follow-up appointments (21.7%) were the other main issues respondents identified regarding NHS support or treatment.

Changing Futures

Presentation by Mark Smith

Minutes:

The Board received a presentation from Changing Futures Northumbria on the progress of their work.

The presentation gave examples of the difficulties that individuals face when interacting with the healthcare system. A study was done with three individuals to understand their increasing interactions with the healthcare system and the cost vs benefits of this. The presentation focused on the journeys of these individuals before and after the intervention of Changing Futures Northumbria, and how this has impacted their journeys.

A distinction was made between doing things better and doing better things; this piece of work is refined around reorientating existing work.

The Chair noted that People@theHeart works on the principals outlined by Changing Futures Northumbria and is co-funded by health organisations across Gateshead.

The Board noted that a barrier to working in this new model is regulations around data protection and consent. Changing Futures have approached this by building networks with the consent of the individual concerned.

The Board received a presentation on the development of a sexual health needs assessment.

Sexual health has been incorporated as a priority area for action in the Council’s Public Health approach to achieve the aims of our Health and Wellbeing Strategy for the Borough of Gateshead. Due to a lack of national direction around sexual health, the North East and North Cumbria leads have come together as a group, “NENC Sexual Health Network of Commissioners and Providers,” and have agreed regional priorities.

The regional priorities are:

  • Give every child the best start in life, with a focus on conception to age two.
  • Enable all children, young people, and adults to maximise their capabilities and have control over their lives
  • Create the conditions for fair employment and good work for all
  • Ensure a healthy standard of living for all, in accordance with international law on economic and social rights
  • Create and develop sustainable place and communities
  • Strengthen the role and impact of ill health prevention

The presentation reviewed significant trends identified in rates of STI per 100,000 people in Gateshead and in England as a whole.

A survey and focus groups were delivered, which brought results on location of services, misconceptions, culture & stigma, and engagement with services, which were presented to the Board.

The recommendations of the sexual health needs assessment are as follows:

  • Wider system working – establish a Sexual Health Reproductive Health Network
  • Service access – enhance the service offer (clinics, locations and digital offer)
  • Promotion – tailored service promotion at mainstream venues
  • Contraception – enable greater and targeted LARC access in the community
  • STIs – support STI action plans and enhance the condom offer
  • Targeted delivery – sexual health in operating models and culturally accessible main service offer to all
  • Education – gain an understanding of the health behaviours of children in the Borough to inform support/offer
  • Research – gather qualitative data on populations at right risk behaviours

The Board were informed of the immediate next steps happening because of this work.

The Chair recommended that Councillors are able to inform about youth events happening in local areas.

The Board acknowledged that sexual health services are siloed in health provision and would like to see them more centrally brought into services. Previously, the Lily System was in place, and a Lily Hub is being piloted and is being linked with labs at Queen Elizabeth Hospital to help ground services more locally.

The Board requested that this work linked into the alcohol strategy due to connections with STI diagnosis and alcohol consumption.

The Board received an update from the Gateshead Cares System Board.

An in-depth update was provided on Priority 1: Giving Children and Young People the Best Start in Life.

  • Recovery Plan for the SALT service has been implemented following the SEND inspection in May – including ICB funded ‘Elklan’ training (SALT upskilling for teaching staff) to support early intervention
  • Funding provided to employ two autism care co-ordinators to reduce waiting times for autism assessment clinic, who will work closely with clinical teams to gather comprehensive evidence of child’s needs during waiting period – positive response from parents receiving diagnoses on same day as assessment.
  • Core20Plus5:-continue to raise health awareness in educational settings, focusing on conditions like asthma, allergies, and diabetes, and has provided masterclasses to empower staff in supporting affected children.

The following further updates were given to the Board on ongoing work:

  • Enabler: Workforce (CBC Update)
  • Compassionate Communities programme of work
    • Including proposed outcomes and proposed funding/resource bids
  • Women’s Health Hubs
  • Great North Healthcare Alliance

  • Gateshead Care’s response for ‘Change NHS’ consultation

The Board noted that there will be a launch event at the end of the year for Change NHS which will be circulated closer to the time.

The Board received a report and oversight of the Better Care Fund 2024/25 Quarter 2 Report, including the discharge fund for Gateshead and the performance for the first 6 months of 2024/25.

The return has highlighted that all four metrics are currently on track to meet the targets for 2024/25. Investment in services to support avoidable admissions to hospital and improve flow out of hospital is showing a positive position compared to previous year.

The activity for Pathway 1 to 3 services in the first 6 months had been less than the demand projections for most areas. Capacity to support discharge is available should demand increase to projected levels for the remaining 6 months

The activity to support people in the Community to prevent hospital admissions is higher than the projected demands. This demonstrates the shift in focus to prevent admissions instead of focusing mainly on hospital discharges is starting to decrease the demand for Pathway 1 to 3 services.

Considerations as to whether we will consider changing our demand forecasts will be made following the Quarter 3 return.
